Community Space Usage Request

This agreement is to set the terms and conditions of use of the McIndoes Academy Community Center facility by our Community Partners. When utilizing our facilities, we are excited to help promote your programs and, in exchange, we hope you recognize that you are representing our organization. This agreement is to help us form a mutually beneficial partnership. Please complete this form in its entirety.

Community Space Terms & Conditions

  • There is no charge for the space for qualified Community Partners. 
  • Community Partner agrees to carry any additional insurance as needed for their program and will have received any necessary training and/or certifications to conduct their program safely and in accordance with all laws and regulations.
  • The Community Partner will be held responsible for any and all guests on the premises before, during, and after the event.  Interior space occupancy is limited to 25 people.
  • Community Partner will be permitted to enter the facilities before the reserved time frame for preparation, facility inspection and equipment drop off.  It is expected that the Community Partner will be in the facility at least 15 minutes prior to the program to ready the space and greet participants.
  • Community Partner events can, by request, be promoted on the Academy website and Facebook page. Community Partner is responsible for additional promotion to ensure the event's success.
  • All activities including basic cleanup (properly dispose of food and beverages, wipe down tables and chairs, wash, dry and put away dishes, sweep or vacuum if needed) shall conclude no later than one hour after event end.  Trash should be taken off premises and disposed of properly. 
  • For events where alcohol will be served, please contact an Academy representative to discuss details and providing a required certificate of insurance.
  • All guests attending the event shall park within the designated areas and not on the lawns of adjacent neighbors. 
  • McIndoes Academy Community Center will not be held liable for any property left on the premises after the program’s conclusion.
  • McIndoes Academy Community Center staff will be permitted to enter the premises at any time during the program.
  • McIndoes Academy Community Center staff will be permitted to photograph the event so long as such activity does not disrupt the program.
  • No smoking, marijuana or illegal substances will be permitted in any area of the facility at any time.
  • Public events or activities put on by various local entities for public benefit are at no cost. Private events and events for profit are at a suggested donation of 1 hour --- $25, 1/2 day --- $75, full day --- $100.
